Liability Waiver

Assumption of Risk – You, as parent and/or guardian of your child(ren) are aware of the inherent risks of injury, death, and property damage involved in sporting activities including, but not limited to, ice hockey, etc. You as a parent and/or guardian of your child(ren) are aware of the risks of injury, death, and property damage that may result from, among other causes, the act of active and passive negligence of Sharks Elite Hockey LLC, all officers, trustees, employees, agents, representatives, successors, and assigns, (collectively, “related parties”), including without limitation the risk of negligent instruction or supervision.

Release of Liability – You as parent and/or guardian of the child(ren) hereby release the related parties from all liability to you as parent of guardian, and or child(ren), principals, employees, agents, representatives, guardians, successors, assigns, heirs, and next of kin from all liability, claims, damage, or demands from personal injury, death, or property damage, arising from or related to this agreement or to camp activities including, but not limited to, ice hockey, etc. whether the injury, death, or property damage occurs on or off the premises. This release includes, without limitation, any personal injury, death, or property damage caused by the active or passive negligence of any of the released parties. You as the parent and/or guardian of the child(ren) bear sole responsibility for any loss.

You as the parent and/or guardian shall indemnify, defend, and hold harmless Sharks Elite Hockey LLC, all officers, trustees, employees, agents, representatives, successors, and assigns, (collectively, “indemnity”), against all liability, demands, claims, costs, losses, damages, recoveries, settlements, and expenses (including interest, penalties, attorney’s fees, expert witness fees, costs, and expenses incurred by indemnity (“losses”), known or unknown, contingent or otherwise directly or indirectly arising from or related to your child(ren)’s participation in Sharks Elite Hockey LLC activities.
